Job summary

Qualifications

Georgia LPN license required.
Current CPR certification.
Experience in long-term care preferred.

Responsibilities

Conduct assessments and develop individualized care plans.
Administer medications and treatments as ordered.
Monitor vital signs and resident condition changes.
Collaborate with the interdisciplinary healthcare team.
Assist with admissions, discharges, and transfers.
Provide education and support to residents and families.
Ensure compliance with nursing standards, safety, and facility policies.
Demonstrate compassion, empathy, and professionalism with high-acuity residents.
