Job summary

Vestal Park Rehabilitation and Nursing Center is seeking dedicated Home Health Aides (HHAs) and Personal Care Aides (PCAs) to join our compassionate team. You will provide care to residents, promoting independence and well-being during the 7pm-7am shift.

Responsibilities include assisting with daily living activities, taking vital signs, managing documentation, and ensuring safety. Candidates must have an HHA or PCA certification and be at least 18 years old.

Qualifications

• HHA Certificate or PCA required.
• Minimum age 18 years.
• Ability to read, understand, and follow resident care plans.
• Strong communication and interpersonal skills.

Responsibilities

• Administer medications and treatments as directed and document accordingly.
• Assist residents with activities of daily living including personal care and hygiene.
• Measure and record vital signs as ordered by providers.
• Assist with exercises, oxygen equipment, and ostomy care as needed.
• Perform administrative tasks and complete forms/reports.
• Participate in facility surveys and inspections as required.
• Respond to call bells and provide personal assistance as needed.
• Assist with activities, mealtime, and resident orientation.
• Notify supervisors of changes in resident status or medical needs.
• Maintain confidentiality and adhere to policies and procedures.

• Administer medications and treatments as directed by physicians and update associated documentation
• Assist residents with activities of daily living, including personal care and hygiene
• Provide vital sign measurements and record vital signs as ordered by providers
• Assist residents with exercises, oxygen equipment, and ostomy care as needed
• Perform administrative tasks, such as completing necessary forms and reports
• Participate in facility surveys and inspections as required
• Provide personal assistance and respond to call bells or signaling calls
• Assist with activities, mealtime, and resident orientation
• Notify supervisors of any changes in resident status or medical needs
• Maintain confidentiality and adhere to all facility policies and procedures

Requirements:

• Current and valid HHA Certificate or PCA
• At least 18 years of age
• Ability to read, understand, and follow resident care plans
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Reliable transportation and willingness to travel to resident locations (if applicable)
