Job summary

Qualifications

• PCA or HHA certification must be current

Responsibilities

• Assist clients with personal care activities (bathing, grooming, dressing, mobility)
• Assist with light household tasks (meal prep, light cleaning, laundry)
• Ensure client safety and proper use of equipment
• Document services accurately per agency policies
• Report changes in client condition to RN Supervisor
• Maintain confidentiality and professional conduct
• Adhere to dress code and hygiene standards
• Demonstrate reliability, punctuality, and commitment to quality care
• Participate in supervisory and care team processes per NY regulations
• Perform additional duties as assigned

Responsibilities

• Assist clients with personal care activities, including bathing, grooming, dressing, and mobility assistance
• Provide support with household tasks such as meal preparation, light housekeeping, and laundry
• Ensure client safety and proper use of equipment, including wheelchairs, oxygen equipment, side rails, and other assistive devices
• Accurately document services provided in accordance with agency policies and procedures
• Report changes in client condition, incidents, or concerns to the RN Supervisor promptly
• Establish and maintain positive, professional relationships with clients and their families
• Maintain the confidentiality and integrity of client information and records
• Adhere to agency dress code, personal hygiene standards, and professional conduct expectations
• Demonstrate reliability, punctuality, and commitment to quality care
• Participate in supervisory and care team processes as required by New York State regulations
• Perform additional duties as assigned by the supervising nurse
