Job summary

ArchCareCareers Home Health Aide provides health-related tasks and assistance with personal hygiene, and activities of daily living in the client’s home or facility under RN supervision.

Responsibilities include carrying out the agency’s mission, participating in care plan development, and following care plan instructions as directed by the RN. Current NYS DOH approved training or equivalency program is required.

Qualifications

• Current NYS DOH approved Basic Training in Home Health Aide or Equivalency Exam.
• Competency-only programs accepted in Home Health Aide Services.

• Provides personal care and rehabilitative services as directed by RN or DCS.
• Follows care plan instructions as directed by RN.

Responsibilities:

The Home Health Aide is responsible for performing health related tasks and may also provide assistance with personal hygiene, housekeeping and other related supportive tasks to a client in his/her home or facility under the supervision of a registered nurse.

• Carries out the agency's mission, philosophy, goals and objectives within guidelines of Agency policy and position function.


• Participates in the development and implementation of a realistic goal-oriented plan of care.


• Provides personal care and rehabilitative services and assists in activities of daily living as directed by the Director of Clinical Services or the supervising Registered Nurse.


• Follows care plan instructions as directed by Registered Nurse.

Qualifications:

• Current NYS DOH approved Basic Training in Home Health Aide OR Equivalency Exam or Competency Only Program in Home Health Aide Services
