Job summary

archcarecareers is seeking a Home Health Aide to provide health-related tasks and assist with personal hygiene, daily living activities, and light housekeeping under the supervision of a registered nurse in a client’s home or facility.

Responsibilities include carrying out the agency’s mission, participating in care planning, delivering personal care and rehabilitative services, and following care plan instructions from the supervising RN. NYS DOH training or equivalency is required.

Qualifications

• Must have NYS DOH approved Basic Training in Home Health Aide or equivalency
• Ability to provide personal care and assist with activities of daily living under RN supervision

Job description

• Carries out the agency’s mission, philosophy, goals and objectives within guidelines of Agency policy and position function.
• Participates in the development and implementation of a realistic goal-oriented plan of care.
• Provides personal care and rehabilitative services and assists in activities of daily living as directed by the Director of Clinical Services or the supervising Registered Nurse.
• Follows care plan instructions as directed by Registered Nurse.

Current NYS DOH approved Basic Training in Home Health Aide OR Equivalency Exam or Competency Only Program in Home Health Aide Services
