Job summary

The Lakes Home Care Services is seeking a part-time home health aide to support patients at home in Florida. Responsibilities include assisting with daily living activities, following the plan of care, and ensuring patient safety.

CPR certification is required, along with state-mandated training and Maxim competency program completion. The role emphasizes clear communication with clients and caregivers, accurate documentation, and adherence to safety practices.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program

Responsibilities

• Assist patients with activities of daily living per plan of care.
• Perform temperature, pulse, respiration, and blood pressure readings when ordered.
• Support safe transfers and ambulation.
• Provide exercises according to plan of care.
• Conduct extensional therapy procedures as ordered.
• Handle patient laundry and safety with equipment.
• Observe home safety practices and teach them to patients.
• Recognize emergencies and apply appropriate management procedures.
• Must follow patients’ plan of care and complete all tasks assigned.
• Communicates clearly with client, family/caregiver and office.
• Observes and documents patient condition and reports changes.

Job description

Responsibilities:

• Responsible for assisting patients with and per plan of care, the following:
  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.
  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
  • Safe transfer and ambulation techniques
  • Exercise according to the plan of care
  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
  • Patient’s laundry
  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
  • Observation and teaching of home safety practices
  • Recognition of an emergency situation and knowledge of emergency management procedures
• Must follow patients’ plan of care and complete all tasks assigned
• Communicates in a constant, clear, and effective way with the client, family/caregiver and office
• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor
