Job summary

The Lakes Home Care, Inc. in Naples, FL is seeking a compassionate Home Health Aide to support patients at home according to an established care plan. The role emphasizes daily living assistance, safety, and respectful, person-centered care.

You will work part-time with flexible hours, perform routine tasks per plan of care, document observations, and communicate clearly with the patient, family, and supervising nurse. The Lakes Home Care, Inc.

Qualifications

• CPR certification and knowledge of basic life support.
• Able to read and follow written care plans and document care.
• Completes Maxim competency program and passes screenings.
• Meets all federal, state, and local requirements.

Responsibilities

• Assist patients with daily living activities per care plan (baths, dressing, feeding, etc.).
• Take and record vital signs when ordered (temperature, pulse, respiration, blood pressure).
• Safe transfer and ambulation per plan of care.
• Perform routine ROM and other simple procedures as ordered.
• Document patient condition and report changes to supervisor.
• Maintain safety and dignity, and observe home safety practices.

Job description

The Lakes Home Care is looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

Responsibilities:

• Responsible for assisting patients with and per plan of care, the following:

  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.
  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
  • Safe transfer and ambulation techniques
  • Exercise according to the plan of care
  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
  • Patient’s laundry
  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
  • Observation and teaching of home safety practices
  • Recognition of an emergency situation and knowledge of emergency management procedures
• Must follow patients’ plan of care and complete all tasks assigned
• Communicates in a constant, clear, and effective way with the client, family/caregiver and office
• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements:

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program
