The Lakes Home Care is seeking a dedicated home health aide to support patients at home in Englewood, Florida. You will help with daily activities, monitor vital signs, and follow each patient’s care plan with compassion and professionalism.

This part-time role offers flexible hours, on-site support, and the chance to make a meaningful difference in clients’ quality of life while maintaining independence and dignity in their homes.

Qualifications

• Meets all federal, state, and local requirements where applicable.
• Ability to read, follow written instructions and document care given.
• Must maintain CPR certification.
• Maxim screening/compliance as required.

Responsabilités

• Assist patients with activities per plan of care including baths, oral hygiene, dressing, feeding, and toileting.
• Record vital signs and document changes as ordered.
• Perform transfers and ambulation with safe techniques.
• Support exercise according to plan of care and range of motion as directed.
• Ensure patient safety and home safety practices.
• Communicate clearly with client, family, and supervisor.
• Observe and report patient condition promptly.

Responsibilities:

• Responsible for assisting patients with and per plan of care, the following:
  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.
  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
  • Safe transfer and ambulation techniques
  • Exercise according to the plan of care
  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
  • Patient’s laundry
  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
  • Observation and teaching of home safety practices
  • Recognition of an emergency situation and knowledge of emergency management procedures
• Must follow patients’ plan of care and complete all tasks assigned
• Communicates in a constant, clear, and effective way with the client, family/caregiver and office
• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements:

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program
