Job summary

The Lakes Home Care Services is seeking part-time home health aides to support patients in Punta Gorda, FL. You will assist with daily activities according to each patient’s plan of care, monitor vital signs, and help with safe transfers and exercises in the home setting.

Qualifications

Responsibilities

• Assist patients per plan of care with daily activities
• Record vital signs when ordered
• Safe transfer and ambulation
• Exercise according to the plan of care
• Perform simple procedures as extensional therapy service
• Help with patient laundry
• Observe and document patient condition and report changes to the nurse
• Communicate clearly with client, family, and office
• Promote home safety and education on safety practices

Job description

Responsibilities:

• Responsible for assisting patients with and per plan of care, the following:
  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.
  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
  • Safe transfer and ambulation techniques
  • Exercise according to the plan of care
  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
  • Patient’s laundry
  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
  • Observation and teaching of home safety practices
  • Recognition of an emergency situation and knowledge of emergency management procedures
• Must follow patients’ plan of care and complete all tasks assigned
• Communicates in a constant, clear, and effective way with the client, family/caregiver and office
• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements:

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program
