Job summary

Split Rock Rehabilitation & Health Care Center seeks compassionate Home Health Aides to assist residents with transportation and non-clinical support throughout the facility. You will help residents maintain comfort, dignity, and quality of life during daily activities.

Responsibilities include safely transporting residents to therapy, activities, and appointments; assisting with mobility devices; collaborating with nursing staff; and upholding safety and infection control policies.

Qualifications

• Current New York State Home Health Aide (HHA) certification required.
• Experience in nursing home, rehabilitation center, or healthcare setting preferred.
• Ability to transport residents safely and assist with mobility needs.
• Strong communication and interpersonal skills.
• Reliable, compassionate, and team-oriented.
• Ability to work flexible schedules, including weekends and holidays as needed.

• Follow facility policies regarding safety, infection control, and resident rights.
• Assist with keeping transportation equipment clean and in good working order.
• Support nursing and interdisciplinary teams as needed.
