Job summary

Split Rock Rehabilitation & Health Care Center is seeking compassionate Home Health Aides (HHAs) to assist residents with transportation and non-clinical support services throughout the facility. This role is ideal for individuals who enjoy working with seniors and helping residents maintain dignity and quality of life.

Qualifications

• HHA certification in NY state is required.
• Experience in a nursing home, rehab center, or healthcare setting is preferred.
• Ability to assist residents with mobility and transportation.
• Strong communication and interpersonal skills.
• Reliable, compassionate, and team-oriented attitude.
• Able to work flexible schedules including weekends/holidays.

Responsibilities

• Safely transport residents to therapy, activities, dining, appointments, and other areas.
• Assist residents with wheelchairs, stretchers, and mobility equipment.
• Provide comfort and non-clinical support during transport.
• Escort residents to activities and appointments as directed.
• Observe and report changes in resident condition to nursing staff.
• Maintain courteous, professional, resident-centered approach at all times.
• Follow facility policies on safety, infection control, and resident rights.
• Keep transportation equipment clean and in good condition.
• Support nursing and interdisciplinary teams as needed.
