Job summary

Hemophilia Outreach of WI Inc. seeks an Advanced Practice Provider (APP) to join our multidisciplinary team in Green Bay. You will evaluate and manage children and adults with bleeding disorders, coordinating individualized treatment plans and promoting continuity of care in collaboration with hematologists and the care team.

Prior hematology experience is preferred but not required. A Master’s in an NP or PA program, Wisconsin licensure with national certification, and the ability to care for

Qualifications

• Master’s degree or higher (NP or PA) from an accredited program.
• Wisconsin licensure and national board certs for APP discipline.
• NPs must be qualified for pediatric and adult care.

Responsibilities

• Perform histories, exams, and assessments for pediatric and adult bleeding disorders.
• Develop and adjust individualized care plans with hematologists.
• Coordinate hemostatic treatment for surgeries, dental procedures, and pregnancies.
• Participate in clinic visits, case discussions, and care transitions.
• Educate patients and caregivers on diagnoses, home therapy, bleeding prevention, and self-management.
