Job summary

Innovative Hematology, Inc. (IHI) in Indianapolis seeks an Advanced Practice Provider to medically manage hematology patients in collaboration with IHI physicians, focusing on inpatient and transitional care.

The Inpatient Hematologist APP will triage patient issues, conduct history and exams, order tests, develop treatment plans, educate families, and document in the EMR. CPR and Indiana PA/NP credentials required.

Qualifications

• Master's degree required.
• At least 3 years of related experience.
• Valid Indiana PA/NP license or PA-C certification.
• Current CPR certification prior to start date.

Responsibilities

• Triage and manage inpatient and outpatient issues.
• Manage patients: history, meds, exam, tests, and treatment plans.
• Collaborate with IHI physicians and team members.
• Document in EMR per IHI/Ascension St. Vincent Hospital policies.
• Perform delegated procedures with physician approval.
• Collect outcomes and data for grant reporting.
• Support transition of pediatric to adult care.
• Authorized to administer Basic Life Support per CPR standards.

What You Will Do

The role of the Advanced Practice Provider is to medically manage patients of IHI in collaboration with the IHI physicians.

Inpatient Hematologist APP collaborating with IHI physicians at neighboring hospitals.

The Opportunity

1. Triage/management of patient issues in the inpatient and/or outpatient setting.
2. Patient management including, but not limited to; health history, medication review, physical examination, ordering and interpretation of laboratory and radiology studies, development and implementation of a treatment plan, education/counseling, ongoing monitoring and follow-up.
3. Effective cooperation and communication with other team members to ensure all patient needs are met, including phone triage.
4. Documentation in Electronic Medical Record per policies of IHI and/or Ascension St. Vincent Hospital.
5. Perform procedures, as delegated, with competency check off by the physician.
6. Collect outcomes and data for applicable grant reporting.
7. Work with patients to successfully transition from the pediatric medical providers to adult providers at the appropriate age/developmental level.
8. Authorized to administer Basic Life Support in accordance with current CPR standards.

EDUCATION:Minimum Master’s degree

EXPERIENCE:Minimum 3 years of related experience

OTHER REQUIREMENTS:Valid Indiana Physician Assistant license, Valid Indiana Nurse Practitioner license, or Physician Assistant Certification (PA-C). Valid CPR certification from a nationally recognized organization (e.g., American Red Cross or American Heart Association) prior to start date
