ArchWell Health is seeking a Market Quality Coordinator (Medical Assistant) to help close STARS and HEDIS gaps within an outpatient senior-focused primary care market. You will review records, coordinate patient outreach, and partner with center leadership to drive quality outcomes.

Ideal candidates have a Medical Assistant background, 1+ year of relevant experience, BLS, and EMR familiarity; strong teamwork and HIPAA compliance are essential for success.

Qualifications

• Knowledge of quality measures and gap closure strategies.
• Experience in a primary care setting.
• Proficient with Word, Excel, PowerPoint, Outlook, and Teams.
• Ability to work independently with strong attention to detail.

Responsibilities

• Monitor CMS STARS/HEDIS measures and drive gaps to closure.
• Review medical records to document Focus On Care measures.
• Request records and upload documentation to EMR.
• Conduct patient outreach to schedule screenings and close gaps.
• Coordinate ancillary visits for in-center screenings.
• Meet with Market Leaders to review progress and strategy.
