Job summary

Penn Medicine is seeking a Nurse Practitioner to join the Heart Transplant program at CPUP and HUP. The ADVANCED PRACTICE PROVIDER will manage post-transplant care, including both ambulatory and inpatient care, with a focus on longitudinal follow-up and telemedicine monitoring.

The role requires PA/NP licensure in Pennsylvania, MSN with ACNP or PA training, and BLS/ACLS certification. The position is full-time with weekend rotations and on-call duties in a leading academic medical center.

Qualifications

• Current licensure in Pennsylvania as an Advanced Practice Provider (APN/PA).
• ACNP or PA program graduate with appropriate certification.
• BLS and ACLS certifications are required.

Responsibilities

• Provide post-transplant care and longitudinal follow-up.
• Coordinate ambulatory and inpatient care with a multidisciplinary team.
• Utilize telemedicine for monitoring and follow-up.
• Support pre- and post-operative care planning and patient education.
