Job summary

Memorial Hermann Health System in Houston, TX is seeking an Advanced Practice Provider for the Heart Failure Transplant ICU. You will deliver comprehensive patient care within a top cardiac team and participate in complex post-operative management of heart failure patients.

Position supports NP or PA, FT/Nights, rotating weekends/holidays, with no call. Requires AGACNP or PA credential and 2–3 years critical care heart failure experience; training on devices will be provided.

Qualifications

• 2–3 years of RN, PA or NP Critical Care experience.
• Experience with heart failure population required.
• AGACNP or PA credential required.
• New NPs or PAs must have 2–3 years RN CCU experience.
• Active licensure in Texas as NP or PA.

Responsibilities

• Provide comprehensive patient care with systematic assessment and management.
• Collaborate with interdisciplinary team in the ICU heart failure population.
• Perform rounds, develop and modify treatment plans, and monitor outcomes.
• Educate patients and families and ensure adherence to care protocols.
