Qualifications

• Massachusetts RN license required
• 5+ years of Intensive Care Cardiac Surgery experience required; within the past 2 years
• Transplant patient care experience required
• Ambulatory care experience strongly preferred
• CCTC or ABTC obtained within 2 years of employment

Responsibilities

• Facilitates interactions among multidisciplinary transplant team members.
• Liaises between inpatient and outpatient transplant care teams.
• Communicates patient status updates to referring physicians.
• Triage calls and organize outpatient or inpatient follow-up as needed.
• Enter data into EPIC and heart transplant databases; support data reporting.
