The role requires a BSN, 5+ years in cardiac ICU care, and Massachusetts RN licensure, with transplant-specific experience strongly preferred.

Qualifications

• BSN required.
• 5+ years of Intensive Care Cardiac Surgery experience.
• Transplant patient care experience.
• Ambulatory experience with telephone triage preferred.
• Current RN License in Massachusetts.
• CCTC or ABTC certification within 2 years of employment.

Responsibilities

• Facilitate interactions among multidisciplinary team members, including physicians and social workers.
• Act as a liaison between inpatient care team and outpatient management team.
• Communicate with referring physician teams regarding patients' clinical status.
• Conduct telephone assessments of patients to monitor clinical status and adjust medications.
• Assist in urgent patient triage and document care provided.
• Provide education to transplant patients and families on care aspects.
• Set up consults with specialty providers and follow up on recommendations.
• Manage communication with patients, employers, and pharmacies regarding prescriptions and coverage.
• Meet regularly with medical program director and nurse practitioners for quality improvement.
• Attend weekly clinical meetings and heart transplant clinics as needed.

About This Role

The Post Heart Transplant Nurse Coordinator is a registered nurse responsible for the comprehensive care of pre-transplant and post-transplant patients. This role is ideal for a nurse with cardiac surgery experience who can effectively coordinate care and communicate with a multidisciplinary team.
