Job summary

The Brigham and Women's Hospital, Inc. in Boston, MA is seeking a Post Heart Transplant Nurse Coordinator with a strong cardiac surgery background to oversee ambulatory and in-hospital care for pre- and post-transplant patients.

You will work closely with the transplant team, coordinate data entry into EPIC and transplant databases, and support patients across multiple sites in Massachusetts and surrounding states.

Qualifications

• Graduate of an approved school of nursing with current Massachusetts RN registration.
• Five years or more of Intensive Care Cardiac Surgery experience; within the past 2 years.
• Transplant patient care experience required.
• Ambulatory experience with telephone triage strongly preferred; within the last 12 months.
• Knowledge of discharge and resource planning preferred.
• CCTC or ABTC obtained within 2 years of employment.

Responsibilities

• Facilitates interactions of the multidisciplinary team members, including physicians, social work, transplant pharmacist, nutritionist, and outside consultants.
• Liaises between inpatient care team and outpatient management team, including heart transplant nurse practitioners.
• Communicates with referring physician team to keep them up-to-date regarding patients' clinical status.
• Monitors and triages patient status, coordinates care and follow-up; manages medications in consultation with clinical providers.
• Provides patient/family education on activity, diet, medication adherence, and follow-up.
• Sets up consults with specialty providers and communicates results to patients and providers.
• Collaborates with financial counselor and social work to assist patients/families with financial and housing needs.
