Novant Health Heart and Vascular Institute is seeking a Heart Failure Registered Nurse to join the Charlotte team. The role emphasizes coordinating care for HF patients from referral through end-of-life in a fast-paced, team-focused clinic.

The position offers 4–10 hour shifts, with a float between Mooresville and Huntersville clinics and a requirement to obtain Heart Failure Certification within 12 months of hire.

Qualificações

• BSN preferred; 4-year bachelor's degree in nursing.
• 3 years of telemetry, CCU, or cardiac stepdown experience; minimum RN clinical experience.
• RN licensed in NC or SC; Heart Failure Nurse certification required.
• Heart Failure Certification to be obtained within 1 year of hire.

Responsabilidades

• Collaborate with the Heart Failure team across all patient-care aspects from referral to end-of-life.
• Provide disease self-management education and serve as a resource for HF patients.
• Participate in performance improvement initiatives to enhance care and outcomes.
• Triage patient calls and titrate medications with providers within scope.

What You'll Do

• Collaborate with the Heart Failure team in all aspects of patient care, coordinating care needs of the heart failure population from the time of referral through end-of-life.
• Provide disease self-management education and serve as a resource for heart failure patients.
• Actively participate in performance improvement initiatives to enhance care, processes, and outcomes.
• Triage patient phone calls and collaborate with medical providers to titrate medications within scope of practice.

What We're Looking For

• Education: 4 Year / Bachelors Degree BSN, preferred.
• Experience: 3 years of experience in Telemetry, CCU, Cardiac Stepdown; Minimum of 3 years of Clinical RN experience, required.
• Licensure/Certification: Registered as RN in NC or SC, required. Certification as a Certified Heart Failure Nurse, required.
• Additional Skills Required:
  • Additional Skills/Requirements (required)
  • Demonstrates interdisciplinary leadership skills, superior interpersonal communication skills, and able to communicate patient’s needs to providers.
  • Organization skills, critical thinking skills, and clinical experience/expertise in HF management
  • Ability to work independently on their own as well as collaborative with the interdisciplinary team.
