LifeBridge Health’s Cardiovascular Institute in Baltimore seeks a Heart Failure Nurse Navigator RN to join a collaborative team at Northwest Hospital. You will identify high-risk patients, coordinate their care, provide patient education, and support navigation through health and community resources.

Responsibilities include in-person visits, advocacy for patients and families, and conducting comprehensive nursing assessments to guide disease management, with RN licensure and BLS required, BSN

Qualifications

• Registered Nurse License is required.
• BSN or higher preferred.
• BLS Certification from the American Heart Association is required.

Responsibilities
