University of Utah Health in Salt Lake City is seeking a Registered Nurse to provide comprehensive patient care within a 24/7 health system. The role involves direct patient interaction, education, and coordination with the care team to support safe discharge planning.

The ideal candidate will have three years of general nursing experience, an active Utah RN license (or eligibility via interstate compact), and an RQI credential within 30 days of hire.

Qualifications

• Three years of general nursing experience.
• Current RN license in Utah or eligible for licensure under the Interstate Compact.
• Current RQI Healthcare Provider eCredential within 30 days of hire.

Responsabilités

• Conducts new patient interviews and assesses functional health and educational needs.
• Monitors patient’s lab values, test results and progress.
• Provides patient and family education.
• Coordinates patient discharge needs to ensure smooth transition from hospital to outside agencies.
• Acts as a liaison between patient and other medical providers regarding progress, symptoms and complications.
• Collaborates with multi-disciplinary teams for assessing and evaluating patient care.
• Participates in departmental continuing education and in-service programs.
• Rotating schedules with possible nights, weekends, and holidays to meet staffing needs.
