UofL Health is seeking an RN Navigator to coordinate care for patients with advanced heart failure, including transplant and VAD options. The role emphasizes patient education, self-management support, and interfacing with a multidisciplinary team to ensure timely, effective care.

The RN Navigator will implement care plans, monitor patient status, and triage calls while maintaining comprehensive EMR documentation and ensuring continuity of care across the continuum.

Formación

• Associate degree in nursing (ADN) required; BSN preferred.
• Active Kentucky RN license (or compact license) required.
• Three years of clinical experience required.

Responsabilidades

• Coordinate care across the advanced heart failure spectrum with emphasis on education and engagement.
• Implement and monitor care plans; record patient status and communicate with the multidisciplinary team.
• Triage telephone calls from heart failure patients and act as primary contact for patients/families.
• Collaborate with VAD and Transplant teams to expedite interventions and optimize outcomes.
• Staff the outpatient heart failure clinic and schedule follow-ups.
