Job summary

Cleveland Clinic Health System is seeking an experienced Advanced Practice Provider to join the Heart Failure team in an outpatient setting at the Main Campus. The role focuses on comprehensive assessment, treatment planning, and coordination with a multidisciplinary team.

The ideal candidate will hold MSN with APRN certification, have cardiology experience, and be prepared for a busy, day-shift schedule (7:00am–5:30pm).

Qualifications

• Cardiology experience is REQUIRED.
• Nurse Practitioners should be Acute Care Certified or Adult, Adult/Gero AND Acute Care Certified.
• MSN required upon hire or within 3 years for external hires and internal hires moving to APRN role.
• Graduate of an accredited APRN program.

Responsibilities

• Conduct comprehensive histories and physical exams; order labs and tests as needed.
• Perform and interpret routine diagnostic procedures per hospital privileges.
• Prescribe and monitor medications as allowed by license and policy.
• Educate patients about disease, treatment options, and potential side effects.
• Collaborate with a multidisciplinary team to ensure high-quality patient care.
