Job summary

The Cleveland Clinic is seeking an experienced Nurse Practitioner (Acute Care) or Physician Assistant to join the Heart Failure team at Hillcrest Hospital in Ohio. The role focuses on heart failure ICU and floor care, plus outpatient follow-up.

Relocation for qualifying candidates and a sign-on bonus are offered. Ideal candidates have cardiology rotation experience or equivalent, with a minimum of 3 years in the field.

Qualifications

• MSN required upon hire or within 3 years for external hires and internal hires moving from a non-APRN licensed role to an APRN licensed role.
• Graduate of an accredited APRN program.
• If in neonatal care service area Master’s Degree is required upon hire.

Responsibilities

• Conducts thorough medical histories and physical examinations, initiates labs and tests, and develops a treatment plan in collaboration with the health care team.
• Performs and interprets routine diagnostic procedures as credentialed by hospital Medical Executive Committee.
• Prescribes and monitors medications as allowed by license and Cleveland Clinic policies.
• Educates patients about disease, treatments, and potential side effects.
• Participates in medical rounds and collaborates with nursing and other teams to ensure quality care.
• Refer patients to specialists for consultation or resources when needed.
• Ensures continuity of care by coordinating with multi-disciplinary teams and other specialties.
• Participates in quality monitoring and discharge planning to improve patient outcomes.
• Supports education and professional development of the care team.
• Promotes evidence-based guidelines and may assist in research and data collection.
