Job summary

Endeavor Health is seeking an Advanced Practice Provider (APN/PA) for the Center for Cardiac Health - Heart Failure in Elmhurst, IL. The role is full-time with an outpatient focus, Monday–Friday hours, and a 40-hour week.

You will obtain patient histories, perform exams, order and interpret tests, manage acute and chronic conditions, educate families, and coordinate care with physicians and interdisciplinary teams to optimize outcomes and post-discharge follow-up.

Qualifications

• APN or PA licensure in IL required
• DEA license required
• BLS/CPR certification required
• Cardiology/APN or PA experience required

Responsibilities

• Take comprehensive patient histories and perform physical examinations
• Order and interpret diagnostic tests
• Develop differential diagnoses and treat conditions
• Manage acute and chronic cardiology conditions
• Educate patients and families
• Coordinate care with physicians and interdisciplinary teams for post-discharge follow-up
