Job Description:

Utah Valley Pediatrics is seeking a full-time HealthySteps Specialist to join our pediatric primary care team. This position is a Licensed Clinical Social Worker (LCSW) or Certified Social Worker (CSW) who will bring early childhood development expertise directly into well-child visits, helping our youngest patients (ages 0–3) and their families thrive.

HealthySteps is a program of 0 to 3 that integrates a child development expert into the pediatric care team to provide interventions, referrals, and follow-up for families with young children. The HealthySteps Specialist builds strong relationships with families and providers, increasing the efficiency of the medical system and supporting team-based, comprehensive care. Job Specifics:

  • Shift: Monday to Friday, 9:00am-5:00pm
  • Location: 716 West 800 North, #3000 Orem, Utah. This is an in-person role.
Key Responsibilities:
Preventative and Responsive Care
  • Promotes HealthySteps within the practice to families and providers.
  • Conducts team-based well-child visits before, during, or following the primary care provider; visits focus on monitoring development, social-emotional functioning, and relational health, and providing anticipatory guidance.
  • Conducts short-term consultations for needs regarding development and/or behavioral concerns such as sleep, positive parenting, and picky eating.
  • Maintains a family support line and responds to and tracks call requests within the designated response time.
  • Participates in reflective supervision meetings.
  • May facilitate parent/caregiver groups.
Referrals and Community Collaboration
  • Creates and maintains a community resource directory/database; provides referrals and tracks follow-up to help families connect successfully with community resources.
  • Builds relationships with community providers to support referrals, warm hand-offs, and appropriate referral criteria.
  • Participates, as appropriate, in community-wide early childhood and/or mental health meetings.
Team-Based Care
  • Serves as an active member of the HealthySteps Implementation Team and attends regular team meetings.
  • Coordinates with front office staff to align scheduling with provider well-child visits.
  • Works closely with pediatric primary care providers on care coordination, goal setting, coaching, and education about key aspects of child development.
  • Maintains open communication with all members of the primary care team.
  • Provides consultation and/or facilitates training to medical professionals and practice staff on early childhood development, early relational health/infant mental health, and trauma-informed practice.
Caseload Tracking, Documentation, and Reporting
  • Documents all patient clinical activity and care coordination in the EHR.
  • Maintains databases required to meet HealthySteps fidelity metrics, including internal and external referrals.
  • Tracks caseload to ensure capacity to deliver services within the risk-stratified service delivery model, including monitoring exit criteria and escalating families to higher levels of care as needed.
Professional Development
  • Participates in HealthySteps training (Virtual HealthySteps Institute), annual model refresher courses, and other trainings as needed.
  • Participates in administrative and clinical/reflective supervision meetings.
  • Completes the HealthySteps Competency-Based Professional Development Assessment and maintains an annually updated professional development plan.
Requirements
Essential Skills and Attributes:
  • Current licensure in the State of Utah as a Licensed Clinical Social Worker (LCSW) or Certified Social Worker (CSW).
  • Master's Degree in Social Work, Psychology, Counseling, or a related field.
  • Excellent oral and written communication skills
