Qualifications

  • Master’s degree in Social Work, Psychology, Counseling, or related field.
  • Current licensure as LCSW, CMHC, or MFT in state of practice (or eligible within 1 year).
  • Demonstrated experience in behavioral health care, preferably in collaborative care.
  • Excellent verbal and written communication skills.

Responsibilities

  • Participates in team-based well-child visits to support development and relational health.
  • Provides guidance on development, sleep, feeding, behavior, and parenting.
  • Performs developmental and social-emotional screenings and assessments.
  • Delivers brief consultation and short-term interventions for concerns.
  • Supports families to strengthen caregiver-child relationships and healthy development.
  • Follows up with families to reinforce care plans and engagement.
  • Identifies needs and connects families to community resources.
  • Tracks referrals and ensures successful connections within the clinic.
