Qualifications

• Bachelor's degree and 3+ years in a health-related field.
• Strong project management experience in healthcare settings.
• Effective verbal and written communication with physicians and staff.
• Proficient with word processing and spreadsheets.
• Ability to analyze performance data from multiple sources.
• Able to prioritize multiple priorities and adapt to diverse populations.
• Project management training to be completed within 6 months.

Responsibilities

• Collaborate with leadership to champion standard improvement activities across post-acute providers.
• Lead transformation projects tied to payor initiatives and the triple aim.
• Drive project success using project management tools for planning and tracking.
• Analyze multiple data sources and craft summaries for providers.
• Train staff and partners on program requirements, resources, data tools, and quality standards.
• Prepare proposals, reports, and updates for projects and meet deadlines.
• Engage in learning opportunities and share resources for improvement.
• Build strong relationships with colleagues, leadership, providers and partners.
