Job summary

Enhabit, Inc. is seeking a dynamic professional to manage relationships with physicians, hospitals, and other referral sources.

You will educate partners about Enhabit Home Health & Hospice services and drive referral growth while supporting post-acute care transitions to ensure high-quality patient outcomes. The role requires strong communication, presentation and organizational skills, plus a valid driver license.

Qualifications

• BA/BS in healthcare administration or related field; equivalent experience considered.
• Graduate of approved nursing, therapy or social work program preferred.
• Two+ years in healthcare business development, community relations, marketing or sales.
• Knowledge of federal/state regs governing Medicare home health/hospice operations.
• Valid driver license and automobile liability insurance required.

Responsibilities

• Serve as the primary relationship manager with referral sources to grow referrals.
• Educate referral sources, patients and families on services and program requirements.
• Support post-acute care transitions to improve patient outcomes and satisfaction.
