Job summary

Roanoke Chowan Community Health Center is seeking a Referral Specialist to establish and maintain a patient referral system, schedule appointments, and manage paperwork for clinicians. The role requires coordinating with insurance providers and tracking referrals throughout the patient care process.

The Referral Specialist will work on-site, support RCCHC values, and contribute to the PCMH model of care while ensuring timely communication with patients and staff.

Qualifications

• Graduate of an accredited high school or business school.
• Work experience in a business or medical practice preferred.

Responsibilities

• Schedules referral appointments and contacts patients with reminders.
• Ensures appropriate paperwork is sent to the consulting clinician.
• Maintains a tracking log of referred patients.
• Assists insurance companies with information such as pre-existing conditions and coverage.
• Acts as referral liaison between RCCHC and other healthcare facilities.
• Sets authorization for MRI and CT referrals.
• Ensures scheduling adheres to standardized protocol.
• Participates as an active member of the RCCHC team.
• Maintains compliance with workflows, policies and PCMH accreditation.
• Other duties as required.
