Job summary

Team Select Home Care is seeking a Relationship Manager (RM) to coordinate care and operations for a patient population within the Long-Term Home Health/PDN division. You will report to the Director of Operations and collaborate with the Director of Nursing and field staff.

The role requires a high school diploma (Bachelor’s preferred) and at least 1 year of healthcare management experience, with strong Microsoft Office skills.

Qualifications

• Minimum 1 year management experience in healthcare.
• Proficient in MS Office (Outlook, Excel, Word).
• Knowledge of patient care coordination.

Responsibilities

• Coordinating care and operations for a patient population.
• Manage scheduling, staffing, recruiting, and hiring.
• Oversee referral/intake processes including onsite visits.
• Ensure insurance eligibility and proper authorizations.
• Maintain payroll verification with clinical staff.
• Support DOO in achieving financial metric goals.
• Maintain confidentiality of PHI and company information.
