Job summary

Champion Care is seeking a Central Intake Admissions Nurse to join our team and help connect patients with the right level of care across our skilled nursing and rehabilitation facilities. This role supports clinical intake, admissions, and coordination with hospitals and referral sources.

The position requires experience in clinical intake, skilled nursing admissions, case management, utilization review, or post-acute care, plus excellent communication and organizational skills to move

Qualifications

• Experience in skilled nursing, clinical intake, admissions, case management, utilization review, or post-acute care.
• Strong clinical judgment and attention to detail.
• Excellent communication and organizational skills.
• Ability to manage multiple referrals and priorities in a fast-paced environment.
• Comfortable working with electronic referral portals and insurance authorizations.
• Responsive, organized, and able to keep the admissions process moving.

Responsibilities

• Review incoming referrals for clinical and financial appropriateness
• Evaluate diagnoses, medications, behaviors, functional status, and care needs
• Determine whether our facilities can safely meet each resident’s needs
• Obtain and track insurance authorizations
• Identify clinical, financial, or payer barriers before admission
• Make and document initial acceptance or denial decisions
• Communicate with hospitals, referral sources, and facility leadership
• Match accepted residents with the appropriate facility and available bed
• Monitor census, bed availability, and referral activity
• Help move accepted referrals quickly and efficiently through the admission process
