Job summary

Oakview Health & Rehab is seeking an Admissions Coordinator to manage pre-admission and admission processes, coordinate with families and referral sources, and ensure a smooth transition for new admissions. You will work closely with the Interdisciplinary Team to align patient needs with the most appropriate care settings.

The role requires knowledge of Medicare/Medicaid and long-term care admission requirements, along with proficiency in MS Office.

Qualifications

  • Bachelor’s degree or associate’s degree with experience in admissions or healthcare navigation.
  • Knowledge of Medicare, Medicaid, private pay and third-party reimbursement.
  • Understanding of long-term care admission requirements and processes.
  • Ability to interpret safety rules and maintain documentation.

Responsibilities

  • Coordinate pre-admission and admission processes and obtain pertinent information.
  • Verify that patient room is ready prior to admission.
  • Provide support to patient/family during transition and routine visits to new admissions.
  • Orient patients to the center environment and participate in IDT activities.
  • Maintain communication with central intake and plan outreach for nights/weekends.
  • Develop partnerships with healthcare partners to coordinate appropriate care settings.
