Job summary

Enhabit Home Health & Hospice in Texas seeks an Intake/Referral Coordinator to coordinate patient referrals and support service transitions. You will work with branch and sales teams to ensure smooth intake and adherence to regulatory guidelines.

The role requires a high school diploma or equivalent, 2+ years in health care with 1+ year in home health or pediatrics, and strong Medicare knowledge and customer service skills. A valid LPN/LVN license in the practicing state is preferred.

Qualifications

• High school diploma or equivalent required.
• Two years in health care with one year in home health or pediatrics preferred.
• Experience in customer service.
• Knowledge of Medicare guidelines governing home health or hospice.
• Basic tech skills including mobile device use.
• Licensed LPN/LVN in the state of practice.
• Experience with intake responsibilities is a plus.
