Qualifications

• CMT certification required.
• Minimum of 3 years direct support experience or DDA/healthcare experience.
• Strong understanding of medical terminology and patient care processes.
• Excellent communication and interpersonal skills for diverse stakeholders.
• Proficiency with EHR systems and standard office software; physician medication orders and record prep experience.
• Must pass criminal background check and CJIS background as applicable.
• Must have driving experience with a clear record and vehicle.

Responsibilities

• Act as the primary point of contact for clients regarding medical inquiries and service coordination.
• Facilitate communication between healthcare providers, clients, and internal teams for clarity of information.
• Record patient medical history, vital statistics, and test results.
• Prepare examination rooms and assist during patient visits.
• Coordinate and schedule medical appointments, follow-ups, and consultations.
• Interpret and explain medical reports and care plans to clients.
• Monitor cases to ensure timely services and address barriers to care.
• Maintain confidentiality and collaborate with clinical staff on patient information.
