Job summary

Responsibilities

• Promotes agency culture and is an example of outward mindset, seeing people as people.
• Answers phones and addresses caller’s needs/inquiries. Provides general support to visitors.
• Adheres to agency policies and procedures, state and federal rules and regulations.
• Responsible for submitting and obtaining orders, following orders management process, and ensures orders are received from providers with proper signature/date and within 30-day period.
• Serves as a liaison to team regarding client records and other secretarial needs.
• Accepts responsibility for regular attendance and punctuality and fulfills job requirements without regard to time involved.
• Assists team with patient referrals and coordinates with Clinical Director on selection of care team.
• Assists team with tracking recertifications, transfers, resumptions, and discharges, as outlined in care coordinator daily responsibilities.
• Ensures security, integrity, and confidentiality of data, and follows HIPAA guidelines.
• Conveys information to clinical care team effectively using good verbal or written communication to facilitate sharing of information.
• Provide physicians, clients, family members, and other callers with information about agency services.
• Completes chart review on newly admitted patients and pending discharges, alerts team to ensure prior authorization is obtained in accordance with the agency’s prior authorization process.
• Completes Care Coordinator day to day responsibilities timely.
• Assists Clinical Director in preparation of IDG conference, and acts as scribe.
• Completes intake of new referrals timely as outlined by agency intake process.
• Completes any other duties as assigned.
