Job summary

Sage Mountain Senior Living in Thousand Oaks, CA is seeking a compassionate Health & Wellness Director to lead the clinical team and ensure exceptional resident care. As a licensed nurse, you will mentor staff, oversee assessments, care plans, and clinical documentation, and partner with residents, families, physicians, and community leaders to maintain the highest standards.

This full‑time role requires strong leadership, excellent communication, and a commitment to resident safety and quality

Qualifications

• California RN or LPN license required.
• Minimum 1 year experience with seniors.
• 2+ years leadership/management experience.
• Strong clinical assessment and medication management knowledge.
• Excellent communication and organizational skills.
• Willing to work evenings, weekends, and on-call as needed.

Responsibilities

• Lead wellness team and foster resident-centered culture.
• Oversee resident assessments, care plans, and documentation.
• Partner with residents, families, physicians, and community leaders.
• Ensure compliance with state regulations and Agemark policies.
• Supervise medication management and nursing services.

At Sage Mountain Senior Living, an Agemark Senior Living community, we're looking for a compassionate and experienced Health & Wellness Director to lead our clinical team and ensure exceptional care for every resident. What You'll Do

As our Health & Wellness Director, you will:

• Lead and support the wellness team while fostering a positive, resident-centered culture.
• Oversee resident assessments, care plans, and clinical documentation.
• Partner with residents, families, physicians, and community leaders to ensure outstanding care.
• Ensure compliance with state regulations and Agemark policies.
• Supervise medication management and nursing services.
• Hire, coach, train, and develop wellness team members.
• Respond to resident concerns and emergencies with professionalism and compassion.
• Identify opportunities to improve resident health, safety, and quality of life.
• Serve as Manager on Duty as needed.

What We're Looking For

• Current RN or LPN license in California.
• Minimum one year of experience working with seniors.
• Two or more years of leadership or management experience.
• Strong clinical assessment and medication management knowledge.
• Excellent communication, leadership, and organizational skills.
• Ability to build relationships with residents, families, and team members.
• Comfortable using computer systems and electronic documentation.
• Flexibility to work occasional evenings, weekends, and on-call responsibilities as needed.
