Sage Mountain Senior Living, an Agemark Senior Living community in Thousand Oaks, CA, seeks a Health & Wellness Director to lead our clinical team and ensure exceptional resident care. If you're a licensed nurse who mentors others and creates meaningful relationships, we'd love to meet you.

As Health & Wellness Director, you will oversee assessments, care plans, documentation, compliance, and nursing services; hire and develop staff; and partner with families, physicians, and community leaders

Qualifications

• Current RN or LPN license in California (CA).
• Minimum one year of experience working with seniors.
• Two+ years of leadership or management experience.
• Strong clinical assessment and medication management knowledge.
• Excellent communication, leadership and organizational skills.

Responsibilities

• Lead wellness team and foster a resident-centered culture.
• Oversee resident assessments, care plans, and clinical documentation.
• Partner with residents, families, physicians, and leaders to ensure outstanding care.
• Ensure compliance with state regulations and Agemark policies.
• Supervise medication management and nursing services.
• Hire, coach, train and develop wellness staff.
• Respond to resident concerns and emergencies with professionalism and compassion.
• Identify opportunities to improve resident health, safety and quality of life.
• Serve as Manager on Duty as needed.

What You'll Do

What We're Looking For

• Current RN or LPN license in California.
• Minimum one year of experience working with seniors.

• Strong clinical assessment and medication management knowledge.
• Excellent communication, leadership, and organizational skills.
• Ability to build relationships with residents, families, and team members.
• Comfortable using computer systems and electronic documentation.
• Flexibility to work occasional evenings, weekends, and on-call responsibilities as needed.
