Castlewoods Place Assisted Living and Memory Care in Brandon, MS seeks an LVN/LPN Health and Wellness Coordinator. You will ensure implementation of resident care policies, supervise services, conduct assessments, and collaborate with families and contracted providers to deliver comprehensive care.

In this role you will develop service schedules, update plans, and coordinate life enrichment activities while supporting residents with compassion and integrity in a team-focused environment.

Qualifications

• Must be LVN or LPN.
• Implements policies related to resident care.
• Supervises and assists in provision of resident services.
• Conducts assessments, negotiates service plans, and updates service information.

Responsibilities

• Ensure residents are assessed timely and charged accordingly.
• Coordinate life enrichment programming.
• Lead coordination with independent health care providers as requested by residents and families.
