Job summary

Care & Rehab in Boscobel, WI is seeking a Health Unit Coordinator / CNA to join our resident-focused team. You will coordinate admissions, transfers, and discharges while supporting patients and families, and collaborate with the care team to ensure smooth communication and continuity of care.

You will also assist with clerical tasks, provide delegated direct patient care as assigned by an RN, and manage nursing schedules, posting them at least two weeks in advance and handling time-off requests

Qualifications

• Preferred: Prior experience as both a Health Unit Coordinator and a CNA.
• Strong communication, organization and multitasking skills.
• Compassion and dedication to supporting patients, families, and staff.

Responsibilities

• Coordinate admissions, transfers, and discharges while supporting patients and families.

Job Summary

As a HUC/CNA, you’ll be an essential part of our nursing department. You’ll work under the supervision of the Director of Nursing to:

• Coordinate admissions, transfers, and discharges while supporting patients and families.
