Job summary

Care & Rehab (WI & MN) -Ostrander MN, Barron, Boscobel, Cumberland, Ladysmith & Neillsville WI is seeking a Health Unit Coordinator / CNA for a full-time day shift. The role blends clerical coordination with hands-on patient care under the supervision of the Director of Nursing.

You will coordinate admissions, transfers, and discharges, support the care team, and handle scheduling and supply needs to keep operations smooth and compliant.

Qualifications

• Preferred: Prior experience as both a Health Unit Coordinator and a CNA.
• Strong communication, organization, and multitasking skills.
• Compassion and dedication to supporting patients, families, and staff.

Responsibilities

• Coordinate admissions, transfers, and discharges while supporting patients and families.
• Collaborate with the healthcare team to ensure smooth communication and continuity of care.
• Assist with clerical activities and acquisition of materials needed for daily operations.
• Provide delegated direct patient care activities as assigned by the RN.
• Manage nursing schedules: create and post schedules at least 2 weeks in advance, process days-off requests, replace call-ins, and track call-in details while preventing overtime.

Job Summary

As a HUC/CNA, you’ll be an essential part of our nursing department. You’ll work under the supervision of the Director of Nursing to:

• Coordinate admissions, transfers, and discharges while supporting patients and families.
