Job summary

Coconino County Health Start Program seeks an experienced social services professional to perform case management for low-income families. You will interview applicants, assess needs, develop case plans, and coordinate with partner agencies to provide home visits and education.

The role emphasizes working with diverse populations, maintaining confidentiality, and ensuring clients access available resources. An Arizona driver’s license is required to hire and maintain employment.

Qualifications

• Four years of progressively responsible experience in social services.
• Associate degree in a related field with three years of social services experience.
• Bachelor's degree in a related field with two years of social services experience.

Responsibilities

• Interviews and elicits pertinent information from applicants for assistance.
• Conducts comprehensive assessments to identify needs and strengths.
• Interprets eligibility rules for different programs.
• Develops individualized case plans with goals and strategies.
• Provides referrals and information about other services and rights.
• Maintains funding allocations and tracks expenditures.
• Provides follow-up and reviews case plans as needed.
• Coordinates services with other agencies and conducts presentations.
• May conduct home visits as required.
• Compiles data for records and reports.
