Coconino County is seeking an experienced Social Services Case Manager for the Health Start Program in the northern region. Under general supervision, you will perform moderate-difficulty case management for low-income clients, including intake, assessments, developing individualized case plans, and referrals.

The Health Start team includes a Program Coordinator and two full-time Case Workers; the role may involve home visits, data reporting, and coordinating services with other agencies.

Competenze

• Four years of progressively responsible experience in social services work.
• Associate degree plus three years of social services experience.
• Bachelor's degree plus two years of social services experience.
• Any equivalent combination of education, training and experience.

• Interview and elicit pertinent information from applicants for assistance.
• Conduct comprehensive assessments to identify strengths and needs.
• Determine eligibility for a range of programs and explain rules.
• Develop individualized case plans with goals and success strategies.
• Provide referrals and information about other services and rights.
• Support clients in meeting goals and maintain case plans.
• Maintain and manage funding allocations and data reporting.
• Coordinate services with other agencies and present information.
• May conduct home visits and prepare records and reports.
• Attend required meetings and trainings.
