Job summary

Are you someone who can provide advocacy and support to individuals served in learning social and self-care skills?If yes, Lakemary is looking for a Health Service Specialist for our Adults Health Service Program. This position is responsible for the transporting of persons served to medical appointments and advocating and communicating the persons needs to the community provider as well as assisting in the Health Service office to help facilitate and efficient and effective environment. This role functions under general supervision of a Nurse Practitioner.Health Service Specialist Responsibilities:Assists with transporting individuals served to appointments, communicating and advocating for the person as well as assisting with procedures as needed.Remains attentive and a positive representative of Health Services and Lakemary Center throughout the health appointment.Obtains necessary medical documentation and forms to assist community providers with care of individuals served.Submit documentation from appointments and assist in the implementation, follow-up and communication of data to appropriate personnel.Provide direct care to individuals served as assigned by supervisor and assist in the clinic area with minor procedures and diagnostic testing as needed.Assists with maintaining calendars and schedules and routes appointment information to the appropriate areas; coordinates transportation needs for the schedule.
