Job summary

Sun Health's Grandview Terrace in Sun City West, AZ, is seeking a Health Services Nurse to provide professional nursing support to residents, promote safety, wellness, and independence, and respond to routine and emergency needs.

You will collaborate with residents, families, and interdisciplinary teams to support health monitoring, fall prevention, and transitions of care while upholding compassionate, person-centered care.

Qualifications

• Must be LPN or RN with a valid Arizona nursing license.
• Finger Print Clearance Card (Level 1) or ability to obtain.
• Two years of experience working with seniors or related field.
• CPR certification required.

Responsibilities

• Demonstrates sound nursing judgment in routine and emergency situations.
• Builds caring, professional, and respectful relationships with residents.
• Provides basic first aid and performs CPR as appropriate.
• Conducts well checks after hospital discharges or as requested.
• Oversees fall prevention programs in collaboration with health teams.
• Communicates emergencies or health changes promptly with residents or families.
• Provides daily updates to the Director regarding hospitalizations and care.
• Manages clinic supplies purchasing, stocking, and organization.
• Assists in obtaining physician orders for transfers to higher care.

Your Mission

The Health Services Nurse for Independent Living provides professional nursing support to residents while promoting safety, wellness, and independence. This role requires strong clinical judgment, excellent communication skills, and the ability to respond effectively to both routine and emergency situations. The nurse works collaboratively with residents, families, and interdisciplinary departments to support health monitoring, fall prevention, and transitions of care.

Key Responsibilities

• Demonstrates sound nursing judgment in both routine and emergency situations.
• Builds and maintains caring, professional, and respectful relationships with residents.
• Provides basic first aid and performs CPR as appropriate.
• Conducts well checks following hospital discharges or as requested by residents or staff.
• Oversees the community fall prevention program in collaboration with the Healthy Aging and Fitness departments.
• Communicates promptly and effectively with residents and/or their representatives regarding emergencies or changes in physical or cognitive health.
• Provides daily updates to the Director regarding hospitalizations, wound care, and resident well checks.
• Manages purchasing, stocking, and organization of clinic medical supplies.
• Assists the Director in obtaining physician orders for residents transferring to a higher level of care.
• Attends and actively participates in monthly Healthcare Committee meetings.
• Performs annual wellness check-ins and updates resident health files as needed.
• Attends all mandatory meetings, trainings, and in-service education sessions.
• Performs all other duties as assigned by the Director of Health Services.

Qualifications

• Must be an L.P.N. or R.N. with a valid Arizona nursing license.
• Possess a current Level 1 Fingerprint Clearance Card or can obtain is required.
• Two years of experience working with seniors or related field.
• Must maintain educational requirements and licenses necessary to perform in present capacity including CPR certification.
