Job summary

The Jewish Association Serving the Aging (JASA) seeks an Associate Director of Health Services to lead clinical and operational teams across Care Transitions, Medication Adherence, SDOH, Palliative Care, and Health Home in NYC. This role combines staff supervision, program evaluation, and strategic partnerships to expand health services footprint across New York City.

Qualifications

• Education: IMG or equivalent degree required.
• Language: bilingual English/Spanish required (read, write, speak, understand).
• Leadership: minimum 3 years of managerial experience with staff training.
• Technical: proficiency in Microsoft Office and Google Workspace; experience with EHRs; data analysis and process improvement.
• Sector: background in hospital discharge planning or healthcare settings preferred.
• Travel: able to travel to clients' homes via mass transit; physical navigation of residences.

Responsibilities

• Team supervision and training: supervise CTSs, recruit and train clinical refreshers, substitute as needed.
• Medication adherence oversight: liaison and trainer for adherence initiatives and high-risk members.
• Palliative care management: oversee NY Center for Dignity and Support program quality and targets.
• Health Home integration: drive program growth and integrate Health Home with Care Transitions workflows.
• Partner and business development: manage hospital and health plan relationships, prepare case studies.
• Program evaluation and reporting: conduct evaluations, prepare monthly aggregate reports, analyze data.
