Job summary

Trustwell Living of Raytown is seeking a Nurse Manager/Health Services Director to lead the clinical team for our Assisted Living residents. You will drive safe medication practices, mentor staff, and coordinate individualized care plans to help residents live their best lives.

The role requires an RN license, state-survey readiness, and leadership experience in a senior care setting. You will collaborate with families and health professionals and may require on-call clinical coverage.

Responsibilities

• Lead the clinical team with accountability and clear goals

• Visit residents at homes or care settings for admissions and readmissions

• Manage clinical staff scheduling, training, and daily tasks
• Provide on-call clinical support as needed

As the Nurse Manager/Health Services Director at Trustwell Living, you'll be the driving force behind exceptional care for our Assisted Living residents. From ensuring safe medication management and infection control to mentoring staff and coordinating personalized care plans, you’ll help create a thriving, supportive environment where seniors can live their best lives.

The successful candidate for this position will have:

• A passion for seniors and enhancing their quality of life
• A valid Registered Nurse (RN) license; Compact license or ability to obtain is encouraged. An unencumbered license is required. An LPN with leadership experience may also be considered.
• Proven clinical leader in an assisted living or LTC environment with at least one year of successful experience as a Wellness Director, Health Care Coordinator (HCC) or Director of Nursing (DON)
• Understand the importance of being state survey ready, every day
• Have a current driver’s license

Essential Functions:

• Be able to lead the clinical team with a sense of purpose and accountability; utilize metrics and established goals.
• Oversees the medication management services, including training and audit of medication administration practices, collaboration with prescribing healthcare providers and pharmacy staff
• Identifies ongoing needs and services of residents through assessment and service plans utilizing the nursing process
• Visit prospective/current residents in their homes or healthcare settings as needed to assess for admission/readmission to the community.
• Clinical liaison with families, residents, healthcare professionals, and team members
• Direct the infection control measures for the community
• Work collaboratively with others on the community leadership team
• Manage and coordinate the work of the clinical staff. This includes overseeing and/or completing required training, staff schedules monthly and daily work assignments.
• Provide on-call clinical support as needed.
