Job summary

Trustwell Living of Raytown is seeking a Nurse Manager/Health Services Director to drive exceptional care for Assisted Living residents. You will oversee medication management, infection control, and personalized care plans while mentoring staff and coordinating a thriving clinical team.

The successful candidate will be an experienced RN with leadership skills, a valid license, and a drive to maintain state survey readiness in a supportive, resident-focused community.

Qualifications

• RN license required with an unencumbered status; compact license encouraged.
• Proven clinical leadership in an assisted living or LTC setting.
• Experience as Wellness Director, HCC, or DON preferred.
• State survey readiness and valid driver’s license required.

Responsibilities

• Lead the clinical team with accountability and established metrics.
• Oversee medication management services and collaborate with providers.
• Assess resident needs and coordinate care plans using nursing process.
• Visit prospective/current residents for admissions assessments.
• Serve as clinical liaison with families and healthcare professionals.
• Direct infection control measures and work with community leadership.
• Coordinate staff training, scheduling, and daily assignments.
• Provide on-call clinical support as needed.

As the Nurse Manager/Health Services Director at Trustwell Living, you'll be the driving force behind exceptional care for our Assisted Living residents. From ensuring safe medication management and infection control to mentoring staff and coordinating personalized care plans, you’ll help create a thriving, supportive environment where seniors can live their best lives.

The successful candidate for this position will have:

• A passion for seniors and enhancing their quality of life
• A valid Registered Nurse (RN) license; Compact license or ability to obtain is encouraged. An unencumbered license is required. An LPN with leadership experience may also be considered.
• Proven clinical leader in an assisted living or LTC environment with at least one year of successful experience as a Wellness Director, Health Care Coordinator (HCC) or Director of Nursing (DON)
• Understand the importance of being state survey ready, every day
• Have a current driver’s license

Essential Functions:

• Be able to lead the clinical team with a sense of purpose and accountability; utilize metrics and established goals.
• Oversees the medication management services, including training and audit of medication administration practices, collaboration with prescribing healthcare providers and pharmacy staff
• Identifies ongoing needs and services of residents through assessment and service plans utilizing the nursing process
• Visit prospective/current residents in their homes or healthcare settings as needed to assess for admission/readmission to the community.
• Clinical liaison with families, residents, healthcare professionals, and team members
• Direct the infection control measures for the community
• Work collaboratively with others on the community leadership team
• Manage and coordinate the work of the clinical staff. This includes overseeing and/or completing required training, staff schedules monthly and daily work assignments.
• Provide on-call clinical support as needed.
