Job summary

Trustwell Living of Raytown is seeking an Assistant Health Services Director to lead the clinical team in delivering exceptional care to Assisted Living residents. You will oversee medication management, infection control, and staff mentoring to ensure high-quality service.

The role requires an active RN or LPN license, leadership experience in a LTC/assisted living setting, and a current driver’s license. This on-site position emphasizes compliance, teamwork, and resident-centered care.

Qualifications

  • A passion for seniors and enhancing their quality of life.
  • A valid Registered Nurse (RN or LPN) license; a Compact license or ability to obtain one is encouraged. An unencumbered license is required.
  • Proven clinical leader in an assisted living or LTC environment with at least one year of successful experience as a Wellness Director, Health Care Coordinator (HCC), or Director of Nursing (DON).
  • Understand the importance of being state survey-ready every day.
  • Have a current driver’s license.

Responsibilities

  • Lead the clinical team with a sense of purpose and accountability; utilize metrics and established goals.
  • Oversee the medication management services, including training and audit of medication administration practices, collaboration with prescribing healthcare providers, and pharmacy staff.
  • Identify ongoing needs and services of residents through assessment and service plans utilizing the nursing process.
  • Visit prospective and current residents in their homes or healthcare settings as needed to assess their suitability for admission or readmission to the community.
  • Clinical liaison with families, residents, healthcare professionals, and team members, and refer them to the Health Services Director when appropriate.
  • Monitor the quality of work performance of Care Partners, Medication Staff, Licensed Nurses, and report to the Health Services Director.
  • Ensure that infection control procedures are known and followed by staff. Directs the infection control measures for the community.
  • Work closely with staff and designated agent(s) to identify Resident problems, concerns, and issues; follows up as needed and communicates the findings appropriately.
  • Oversees and/or completes monthly nursing schedule for nursing staff. Participates in the Manager on Duty program as directed by the Executive Director.
  • Ensures the accurate transcription of physician orders into the treatment and medication record.
  • Assists as directed with move-in and orientation of new Residents and designated agent(s).
  • Reviews Resident service records, treatment, and medication books to assure quality of care and documentation in the medical record is within nursing standards.
  • Verifies physician orders, move-in/move-out information, and initiates appropriate action/follow-up.
  • Supervises the ordering of medications from the pharmacy.
